NEW APPLICANT Passport Requirements:

1)        Birth Cert –  in security paper fr NSO (+ orig Local Civil Registrar copy during appearance at DFA, if nso is not clearly readable)

If born earlier 1945, submit the ff:: non availability of birth record (in secpa) + orig Local Civil Registrar Birth Cert authenticated by NSO

2)     If not born in Region 7, submit 1 evidence of at least 6 mo. current residence in region 7 like employment cert + co. ID, Voter’s cert, Bills addrs to applicant, Driver’s License, SSS  I.D. / E1 form (w/ stated address w/in region7)

3)        Original Baptismal Certificate with seal & / or O.R.

If NO BAPTISMAL, submit any 1 of the ff: a) NBI clearance b) Voter’s Registration Record  c) TOR. However NBI clearance cant be accepted if applicant’s birth was registered late (birth registration date is more than 1 mo. fr. date of birth.

4)     Personal Appearance at DFA for ages 8 y.o & above (to be sched,).Either parent or co traveler (w/ s-p-a) can appear for child below 8  y.o.

5)     Passport size colored picture 3 pcs (digital type with dark blue background. Recommended studio: inside DFA. can be taken/submitted during personal appearance at DFA

6)        Genuine ID (even expired Schl, Voter’s, SSS, Company ID,Driver’s/student license)bring at least 1 during personal appearance. If no acceptable I.D, submit 2 of requirement #3(instead of only one)

7)        For married women, NSO Marriage Contract + orig LCR copy. If husband is a foreigner, submit CFO cert.

8)        For below 18 y.o., Orig passport of mother & father(if parent has no passport or not in country, mother/father shld execute affidavit of consent(&support)notarized by lawyer or Phil Embassy if staying outside Phil

9)        For below 18 y.o., personal appearance of either parent. If no parent can accompany, personal appearance of authorized rep w/ S-P-A + affidavit of  consent(& support) by parent & DSWD clearance for child to travel.

NOTES: all reqmts to be submitted must be orig. NSO copies wl not be returned after processing. Application form wil be provided to u to fill up in our ofc when u hv all the rqmnts. Date of personal appearance wl be mutually agreed by applicant & us based on available slots. It wl be the start of processing leadtime.
