First Call –  

Ø      Dignified Memorial Services Inc. shall be available on a 24 hour call basis and shall promptly answer the call for assistance. 

Ø      Upon notification, Dignified Memorial Services Inc. shall promptly contact the pre-appointed mortuary for the immediate dispatch of transport to secure the release of the body from place of death to the mortuary. 

Ø      Dignified Memorial Services Inc. will send its Family Service Advisor (FSA) to assist the bereaved family. 

Ø      Dignified Memorial Services Inc. will ensure that all the stated provision in the Member's Agreement must be followed for the comfort of the bereaved family.  

PROGRAM PACKAGE: Php 999.00 (Renewable Yearly)

   ü      Retrieval from place of death to our accredited funeral parlor

ü      Embalming (good for 5 days) and other body preparation

ü      High glossed finish wooden casket

ü      Air-conditioned chapel good for 30 persons

ü      Five (5) days viewing

ü      Registration of Death Certificate at the local Civil Registrar’s Office

ü     Securing of a Certified True Copy of Death Certificate (for family) and necessary transfer and burial permits

ü      24 Hour assistance during or duration of the wake

ü      Memory folder

ü      Funeral Hearse for interment
