Basic Requirements:

 

• Must be 21 - 65 years old

• Must be a Filipino citizen;

• Minimum Gross Fixed Annual Income of PhP120,000;

• Must have at lease one landline phone;

• Residence or Office Address must be within Greater Manila Area, Bulacan, Baguio, Cavite.

Documentary Requirements
For Employed

Proof of Identification: Front and back portion  of valid ID with photo and signature 
                                       such as: Company ID; TIN/SSS/GSIS/Pag-ibig or other 
                                       government-issued ID; Philippine Passport; Driver’s License; 
                                       PRC ID;

Proof of income: Photocopy of latest BIR Form 2316 signed by employer’s authorized   
                             representative (for employed) or photocopy of latest ITR and W4 with
                             BIR stamp (for self-employed)

Proof of Billing/Residency: Any utility bills, insurance bill, card bills, etc. (latest billing
                                             statement bearing your name and addressed to your   
                                             residential address.)

For Self-Emloyed

Clear scanned copies of the following:

• Latest credit card statement
• Certificate of business registration
• Last three months’ bank statement and original bank certification.

• Latest Audited Financial Statements (AFS) with BIR or bank stamp AND latest Income 
  Tax Return (ITR) with BIR or bank stamp

3 Sample Signatures

For Applicants with Existing Credit Card:

• Latest Credit Charge slip (not more than 30 days from transaction date) or latest credit card statement of account

• Front portion of other credit card
