Q.How to know whether a secretion is normal?
Abnormal secretion appears yellowish or green and gives a foul smell. Normal secretion is clear or white and its viscosity depends on the stage of the menstrual cycle: during ovulation, it is more watery and less abundant and the volume discharged rises after ovulation. All women will have some secretion and if it is just a recent phenomenon, do not be over-worried and think that this is due to some kind of disease. Secretion is normal unless there is pain, itch, fever or foul smell.
Q.How do we know whether vaginal infection is due to candida (yeast), bacteria or Trichomonas vaginalis?
Vaginal yeast infection normally has the symptom of itching around the vagina. Vaginal bacterial infection, caused mostly by sexual intercourse, is characterized by rashes, a burning sensation in the vagina and sticky vaginal secretion with foul smell. Trichomoniasis, or vaginal infection by a parasite called Trichomonas vaginalis, is also infected through sexual intercourse and is identified by yellowish or greenish secretion. The last two types of infection are treated by using antibiotics, otherwise they may lead to pelvic cavity infection and result in infertility. Trichomoniasis during pregnancy may lead to miscarriage if left untreated.

There are many causes of vaginitis and these include vaginal bruises, foreign body stimulation (uterus support, vagina plug, cotton balls, bandage), corrosive chemicals, contraceptive tools and cream, vaginal cleansing etc. Besides, pelvic cavity infection, adnexitis, endometritis, miscarriage and profuse secretion after childbirth, prolonged uterus bleeding or vaginal surgical bruises etc may disrupt the natural protection function of the vagina. The common pathogens causing the disease are Staphylococci, Streptococcus, Escherichia coli and Proteus mirabilis.Symptoms:
A common symptom is profuse vaginal secretion appearing sticky and in serious cases giving a foul smell. The secretion may stimulate the opening of the urinary tract, causing feeling of frequent micturition, urgent micturition, dysuria and lowering of vagina, a sense of burning pain and fatigue.
Q. Bacterial vaginitis
Due to frequent sexual intercourse or multiple sex partners, the alkaline sperms may remain in the vagina and inhibit the growth of lactic acid bacillus, thus disrupting the healthy acidic environment in the vagina, leading to imbalanced bacterial groups and causing this disease. Bacterial vaginitis does not show symptoms early and initially the leucorrhea discharge is watery, white and foamy, with a foul smell due to the decomposition of bacterial enzymes. During menstruation or sexual intercourse, the smell would be stronger. The patient would experience itching and burning pain in the external genital parts but gynecological examination would not show any inflammation of the vaginal wall and this is the characteristic of this disease. It can lead to inflammation of the fallopian tube and cause infertility. Infection in pregnant woman may cause premature delivery. The baby thus born may suffer from skin disease with the epidermis peeling off and this could often be wrongly regarded as congenital syphilis.
Q. Moldal vaginitis
Causes:(1) About 80% to 90% of Moldal vaginitis cases are caused by Candida albicans infection. This disease is seen mostly among pregnant ladies, diabetics and patients receiving hormonal treatment;
(2) Long-term use of broad-spectrum antibiotics and adrenal cortical hormone may disrupt the organic bacteria chemical groups leading to modal pores;
(3) Serious infectious diseases and other depleting diseases.
Clinical symptoms:
Most patients suffering from the disease would have more leucorrhea discharge which is sticky and appearing like white bean curd or yogurt. The leucorrhea sticks to the inside of nympha and vaginal mucosa and may bleed due to friction. In about 50% of the patients, the leucorrhea is watery or viscous but does not show any white solid structure indicating that the condition is not serious and only the external genital organs are wet.
