Ø  Any two (2) valid IDs with picture and signature (driver’s license, SSS,

voter’s ID, PRC, Philhealth, TIN, Postal ID, or company ID)

Ø  Latest proof of billing address (telephone, electricity, water, cable,

 credit card,  mobile, etc.)

Ø  Three (3) character references preferably with landlines

Ø  Latest invoices showing purchases, payments & credit limit of borrower

from other direct selling company, (if any)
