3.  Scanned copy of one (1) month latest payslips;

Note: For those whose  basic salaries fall below the minimum requirement but with  allowances to reach the gross income, submit your three (3)months latest payslips.

Gross salary income for Metro Manila, Cavite and Rizal Employees must at least be P 15,000. For the rest of the covered areas, gross salary requirement is only P 12,000.

4.  Scanned copy of latest ITR BIR form 2316.

Note: Please make sure that items 51-54 are duly signed by the employer and employee and the BIR Form No. 2316 1s also visible.

5.  Scanned copy of Certificate of Employment (COE) indicating the date you were hired, your status as a permanent employee and your itemized monthly remuneration including regular allowances.

2. Age must be 25-60 years of age.

3. If SELF-EMPLOYED, business must be at least five (3) years in operation.

If PROFESSIONAL, must be practicing for at least three (3) years.

4. Must have a credit card for at least one (1) year.

5. Credit card account must be in good standing for the last two (2) months:

*Outstanding balance must be within 90 percent of utilization.

* No past due.

6. Must have an office land line number and mobile number; Home phone number if applicable.
