Membership Categories:
   Individual : any individual 13-below 60 yrs old

   Family: 2-9 members, 3 months-below 60 yrs old, at least two(2) are between 13-below 60 yrs old

   Group: 10 or more members, 3 months-below 60 yrs old, at least ten(10) are between 13-below
           60 yrs old

   Corporate: at least 11 principal employees (Rates depend on the number of members, separate
           quotation available)
