Sibutramine HCl monohydrate
Indications
Management of obesity & wt loss maintenance in patients w/ an initial BMI of ≥ 30 kg/m2 or ≥ 27 kg/m2 in the presence of other risk factors (HTN, diabetes, dyslipidemia) in conjunction w/ a reduced calorie diet.
Dosage
Initially 10 mg/day. Max dose: 15 mg.

May be taken with or without food
Contraindications
Major eating disorder. Concomitant use w/ MAOIs & centrally-acting wt-reducing agents. Severe renal & hepatic impairment; hypothyroidism. Childn <16 yr. Pregnancy & lactation.

Special Precautions
CV disease, narrow-angle glaucoma, epilepsy, pulmonary HTN, bleeding disorders. Mild to moderate renal impairment. Re-evaluate therapy if the patient has not lost ≤ 2 kg in the 1st 4 wk of treatment. Monitor BP & pulse prior to initiation of therapy & at regular intervals thereafter. May impair ability to drive or operate machinery. Elderly.
Adverse Drug Reactions
Tachycardia, palpitations, HTN, vasodilation. Constipation, nausea, hemorrhoid aggravation, dry mouth, insomnia. Lightheadedness, paresthesia, headache, anxiety. Sweating. Taste perversion. Thrombocytopenia. Mild skin erruptions, urticaria, angioedema, anaphylaxis. Seizures, transient short-term memory disturbance. Blurred vision. Atrial fibrillation. Alopecia, rash. Acute interstitial nephritis, urinary retention. Reversible increase in liver enzymes.
