List of requirements:

1. Application Form fully accomplished.
2. Barangay Clearance/Permit
3. Police Clearance or NBI
4. Proof of Billing
5. Photocopy of two (2) Valid ID

Procedure of Application:

1. Submit application form with complete requirements.
2. Assessment of application form and requirements.
3. Interview.
4. After passing the interview, pay PhP 750.00 for the E&S Marketing Package (brochures, flyers, rates & packages in clearbook, presentation CD, forms, certificate of accreditation, calling cards & tarpaulin)
5. Orientation and product info seminar (3 hours) min of 5 participants.
