For Self-employed: Must be a credit card holder with at least P50,000 credit limit not exceeding 80% utilization of the credit limit.

2. Photocopy of Company ID with picture and signature and with three (3) specimen signatures

3. Photocopy of 1 valid government issued ID ‏

4. SEC / DTI Certificate of Registration

5. Latest ITR with BIR stamp and tax receipt

6. Latest two (2) years Audited Financial Statements

7. Latest six (6) months bank statements

  8. Latest credit card billing statement
