1. Must be 23 to 63 years old
2. Must be employed for at least 1 year in the present company
3. Basic Monthly income of at least P15,000
Employees of Non-Top Corporation
1. Must be 30 to 63 years old
2. Must have a total work experience of at least 3 years & more than 1 yr in present company
3. Basic Monthly income of at least P20,000 if in Metro Manila / P15,000 working outside Metro Manila
Processing time: Around 10 working days
Self-Employed
1. Must be 30 to 63 years old
2. Must be engaged in the same business which has been profitable for the last 3 years.
3. At least P50,000 gross monthly income (subject to change)
Processing time: 10 working days or more
DOCUMENTARY REQUIREMENTS:
Employees of Top 12,000 Corporations:
1. Latest W2 (form 2316)
2. Latest 1 month payslip or Certificate of Employment (COE) with breakdown of compensation
3. Clear photocopy of 2 Valid IDs (front & back)
4. IF APPLICANT HAS NO RESIDENTIAL POSTPAID LANDLINE: Add latest residential utility bill under applicant's name
Employees of Non-Top Corporations:
1. Latest W2 (form 2316)
2. Latest 1 month payslip
3. Clear photocopy of 2 Valid IDs (front & back)
4. Original Certificate of Employment (COE); must be within 3 months old
5. Latest 3 mos. credit card statements (at least 15% repayment rate) / credit limit of at least P50,000 and should be active for at least 1 year.
6. IF APPLICANT HAS NO RESIDENTIAL POSTPAID LANDLINE: Add latest residential utility bill under applicant's name
Self-Employed:
1. Latest 2 years ITR with audited financial statements
2. Latest 6 months bank statements
3. Latest 3 mos. credit card bill (at least 15% repayment rate) + P50,000 credit limit + at least 1 year active
4. Clear photocopy of 2 valid IDs (front & back)
5. DTI Registration Paper (dated January 2005 OR earlier)
6. Current Mayor's Permit
7. Latest Residential Utility bill
