| Adding an Order Form to Your Advertisement - Sign in / Sign Up for a Google Docs account: https://docs.google.com
- After signing in to Google Docs, create a new form by clicking Create new -> Form
- Edit your new form
- Specify the form title
- Specify the form description to make it easier for your clients to fill up the form
- The Question Title will be the field name
- The Question Type will be the field type
- Click Done button to complete the editing of a field. Click on the field to edit it
- Click Add Item and select a Question type to add another field
- You have the option to make the field required
- You can change the Theme or look of your form by clicking Theme: Plain. Then select the Theme in the displayed page and click Apply.
- When done creating your form, click Save at the upper right corner of the screen (no need to do this if the button is displayed as Saved)
- To get the embed code, click More Actions -> Embed (at the upper right corner of the screen)
- Copy the embed code displayed.
- Using the Advanced Editor of your advertisement, click the Embed toolbar icon
and then paste the copied embed code in the displayed dialog window. - Save your advertisement to display it with the form.
Checking your orders - Sign in to Google docs. https://docs.google.com
- Look for the Form in the list of your documents and click it
- This will open a spreadsheet with the responses from users who submitted your order form

- In the displayed dialog window, tick "A use submits a form" and tick "Email - right away" and then click Save

Tips When Embedding an Order Form - Do not forget a field for Contact Details (either mobile number, landline number or email address; better to ask for all)
- When asking information requiring long text input, use "Paragraph Text" as Question Type instead of just "Text" so that a larger text area will be displayed instead of a one line textbox.
- When using the embed code, adjust the height value so that there will be NO vertical scrollbar that will be displayed (trial and error). The user might miss the Submit button if the entire form is not fully displayed. Here is a sample order form embed code with the corresponding height value:

- Use "Select from a list" ONLY IF there is a fixed set of options. If you are expecting that the user can have an answer that is not among the available options, use "Multiple choice" and add "Other" field.
- Remember, it is easier to click on options than to type the answer. So if you can provide options, use "Select from a list" or "Multiple choice" field. The easier for the user to fill up the form, the higher the conversion.

Ask ko lang po, pwede na rin po ba kami mag lagay ng email opt-in form sa ads namin? Like newsletter subscription form? Allowed na po ba ang javascript na ginagamit namin sa paglalagay ng opt-in form? |

Ask ko lang po, pwede na rin po ba kami mag lagay ng email opt-in form sa ads namin? Like newsletter subscription form?
